Until companies are required to report the specifics as to how they measure fraud, we will never know EXACTLY how much it's costing us. Who are the fraudsters?

They're not always organized crime rings; sometimes they're people just like you and me, but they make a poor decision that effects all of us. They can be doctors, lawyers, medical equipment suppliers, body/repair shops, your neighbor, or even a family member.
